New Patients
Please click the button below to submit a New Patient Appointment Request form. We will reach out to you once we get your request within 3 business days (that is our goal).
Please note that we are getting a larger number of requests than we are used to, and know that your request is not going into a big black hole. We will call you back! If you have not heard from us after 3 business days, call or text the office at (757) 453-5508.
Please review the Billing Page of our website prior to submitting a request for information about appointment costs/reimbursement.
Returning Patients
Follow-up patients, please request an appointment below. You will receive an email confirming the requested time and will be contacted by our staff with details about the appointment or if the slot requested was taken. Please specify whether you would like in-person or virtual in the ‘Reason for the Appointment”.
If you are requesting a follow-up appointment for a patient who is under the age of 18, please enter the Date of Birth of the parent/guardian, otherwise it will not allow you to submit an appointment request.
