Out-of-Network Mental Health Care

At Leva Psychiatry

Leva Psychiatry is considered an “Out of Network” provider.  We will give you a receipt (superbill) with a billing code and diagnosis that you can submit to your insurance company for reimbursement upon request. Prescription medications, labs, and psychological testing appointments are still covered by insurance. 

We accept FSA/HSA/Health Savings Cards. We cannot guarantee that you will be reimbursed for the amount. You can call your insurance company and discuss reimbursement prior to scheduling an appointment. We recommend the Reimbursify app for submitting superbills to insurance.

Please note that you may not submit superbills to Medicare or Medicaid. Medicare patients must complete an ABN form prior to being seen. Please contact the office if you need a link to the ABN form.

  • New Patient Visits: $300 (60 minute appointment)
  • 30 minute Follow-up Visits: $150

  • 30 minute No Show Fee: $50 (for missed/canceled appointments without 24 hour prior notice)

GOOD FAITH ESTIMATE/ NO SURPRISE ACT

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
