About Appointments

This sixty (60) minute appointment includes: Psychiatric Diagnostic Evaluation / Assessment with recommendations for treatment and collaboration with your Primary Care Provider, therapist, or prior treatment provider. A detailed history will be obtained in addition to a review of your past medications, past treatments, and any other documents that you bring with you. Medication may be initiated if it is necessary. This is for clients that are new to Leva Psychiatry and have not been seen in the past.
This 30 or 60 minute appointment includes, in most cases, brief supportive psychotherapy in addition to a consultation for medication management. I will conduct a review of any updated medical information as well as a review of treatment response and make adjustments accordingly. Collaboration with other providers will occur if needed. If any testing is ordered, it will be reviewed, including lab work, psychological testing, or other pertinent records brought to my attention.
This visit includes a 30 minute appointment and consists of the same visit type as the follow up visit. You will use HIPAA compliant Audio/Visual software for the visit. Consent forms for telepsychiatry need to be signed in the office prior to the first telepsychiatry visit. You will need to be seen once yearly in the office. Prior to scheduling your first telepsychiatry visit, you must be an established patient and have been seen for the first time in the office. Your prescriptions will be refilled, similar to when you are seen in the office. There are times when in-office visits are more appropriate and if deemed necessary by the provider, you will be informed that the visit needs to be in-person.

Payment

Most major credit cards and cash are accepted. Checks are not accepted as a form of payment. At the time of your visit, your payment will be collected if we have a current card on file. If you have not saved a card on file, please make a payment here.
